Create review form templates
Build reusable performance-review forms and preview them before use.
Open Performance Management → Form Templates. Templates let you reuse a consistent question set across cycles.
Select Create Form Template, name the template and build the form from available blocks. Supported question styles include:
- Open answer
- Multiple choice
- Scale
- eNPS
- Matrix
Layout blocks help organize the form into readable sections. Configure each block's prompt and options, reorder blocks as needed, then save the template.
From the template list you can open a template, preview the reviewer experience, edit it or duplicate it for a related process. When building a performance cycle, select the saved template for the appropriate review type.
